By submitting this Test Request Form, the Client confirms that the information provided is complete and accurate and authorises New World Laboratories to undertake the requested services. The Client acknowledges that testing will be performed in accordance with NWL's Client Service & Commercial Terms, and that additional charges may apply where work varies from the original request or where additional services are required.

The Client declares that the submitted sample(s) are free from asbestos and any other harmful or hazardous substances, to the best of their knowledge.

By submitting this Test Request Form, the Client confirms that the information provided is complete and accurate and authorises New World Laboratories to undertake the requested services. The Client acknowledges that testing will be performed in accordance with NWL's Client Service & Commercial Terms, and that additional charges may apply where work varies from the original request or where additional services are required.
Important — the PDF is NOT attached automatically.

Submitting downloads a PDF copy of this request and opens a pre-addressed email — you must attach the downloaded PDF yourself before sending it. Please check the PDF is correct first, then attach it to the email draft and hit send. If anything's wrong, click Back to return to the Samples step, make your changes, then come back through and submit again.

    Option 1 — Need to submit another request?

    Start another one now — your client details and test methods carry over, only Samples reset (and Project/Location too, unless ticked below). One combined email goes out at the end with every PDF listed.

    Option 2 — Ready to send this?

    To finish sending your request:

    1. Check the downloaded PDF(s) are correct.
    2. An email draft is about to open, addressed to New World Laboratories (with a copy to you and any other recipients you added).
    3. Attach every PDF that's downloaded to your device — none of them attach themselves.
    4. Hit send in your email app.
